• Please note that this web form is ONLY for printing convenience and Information entered are not stored in our system
• Kindly consolidate all the items under the ‘Your Request’ section and categorize each problem individually if you have more than one item.
• Please read our Repair Terms & Conditions, print your signature at the bottom of the form, and include the printed form with the item(s) you are shipping.
• Please do not send us your dust bag and ensure that all belongings that are irrelevant to the service are removed.
• Please ship the parcel with a tracking number and signature upon delivery to the address below:

• Kindly send us the tracking details so that we can add them to our tracklist.
